MPI-AMRVAC setup code and parameter files for the Kelvin-Helmholtz test with
bulk velocity from Vandenbroucke & De Rijcke.

The code should be configured using
$AMRVAC_DIR/setup.pl -d=22 -g=16,16 -p=hd -eos=gamma -phi=0 -z=0

The naming convention is as follows:
 - kh_fixed_fd: 100x100 finite difference test without bulk velocity
 - kh_fixed_hllc: 100x100 finite volume test without bulk velocity
 - kh_fixed_hllc_hires: 400x400 finite volume test without bulk velocity
 - kh_v100_fd: 100x100 finite difference test with bulk velocity v = 100
 - kh_v100_hllc: 100x100 finite volume test with bulk velocity v = 100
 - kh_v100_hllc_hires: 400x400 finite volume test with bulk velocity v = 100
